Revyval Health, LLC – Terms and Conditions
Effective Date: April 20, 2025
These Terms and Conditions (“Terms”) govern your use of the website, patient portal, telehealth services, and communications provided by Revyval Health, LLC (“Revyval Health,” “we,” “us,” or “our”). By accessing or using any of our services—including our website, telemedicine platform, communication tools, and affiliated pharmacies—you agree to be bound by these Terms. If you do not agree, please discontinue use immediately.
1. Services Provided
Revyval Health, LLC provides medically supervised wellness and optimization services, including but not limited to:
Testosterone Replacement Therapy (TRT)
Hormone Replacement Therapy (HRT)
Peptide Therapy
Weight Loss Programs (including GLP-1 medications)
Diagnostic lab testing and consultations
All medical services are provided under the supervision of licensed medical professionals through U.S.-based telehealth platforms and partner pharmacies. Non-medical communications (e.g., customer support or scheduling) are handled by authorized Revyval Health staff.
2. Medical Disclaimer
Revyval Health, LLC does not replace your primary care physician. Our services are intended for wellness optimization and should not be considered emergency or acute medical care. All prescriptions are written only after consultation and review by a licensed provider. Individual results may vary. Do not discontinue or adjust any prescribed medication without consulting your healthcare provider. If you are experiencing a medical emergency, call 911 immediately.
3. User Responsibilities
By using our services, you agree to:
Provide accurate, complete, and current information.
Disclose all relevant health conditions, medications, and history during consultations.
Use our services solely for lawful and personal medical purposes.
Not misuse, share, or falsify medical prescriptions or information.
You are responsible for maintaining the confidentiality of your login credentials and any data shared through our platform.
4. Account and Access
Accounts are for individual use only. Unauthorized access, duplication, or account sharing is prohibited. We reserve the right to suspend or terminate accounts for policy violations, suspected fraud, or non-payment.
5. Fees and Payment
Fees for consultations, lab testing, and prescriptions will be disclosed before purchase. Payments are due prior to services being rendered unless otherwise agreed in writing. We accept credit/debit cards, ACH, HSA, and FSA where applicable. All payments are non-refundable once a prescription, lab requisition, or consultation has been initiated. Subscription, refill, or recurring payments will be billed according to the plan selected by the patient.
6. Prescription Policy
All medications are dispensed through licensed U.S. compounding or retail pharmacies. Revyval Health does not handle or ship medications directly except in states requiring intermediary routing. Revyval Health reserves the right to deny or modify prescriptions based on medical appropriateness, safety, or compliance.
7. Privacy and Data Protection
Your privacy is governed by our Privacy Policy (available here).
We comply with HIPAA and other applicable privacy laws. Your medical information is encrypted and securely stored. We do not sell or share personal or mobile data for third-party marketing. You may opt out of marketing or SMS communications at any time by replying STOP.
8. Prohibited Activities
You agree not to:
Use our site or services for any unlawful or fraudulent activity. Tamper with or reverse-engineer any part of our platform. Provide false information to obtain medication. Harass or abuse staff or medical professionals.
9. Limitation of Liability
To the maximum extent permitted by law: Revyval Health, LLC is not liable for indirect, incidental, or consequential damages arising from the use or inability to use our services. We are not responsible for third-party system failures, shipping delays, or pharmacy-related issues outside our control.
10. Indemnification
You agree to indemnify and hold harmless Revyval Health, LLC, its affiliates, medical providers, contractors, and employees from any claims, damages, or expenses (including legal fees) arising from your use of our services or violation of these Terms.
11. Termination
We reserve the right to terminate or suspend your access if:
You violate these Terms;
You fail to make payments;
We suspect fraud, abuse, or misrepresentation;
We discontinue the service for regulatory or operational reasons.
Upon termination, all outstanding balances become immediately due.
12. Modifications to Terms
We may revise these Terms at any time. Updates become effective upon posting to our website. Continued use of our services after any modification constitutes acceptance of the updated Terms.
13. Governing Law
These Terms shall be governed by the laws of the State of Florida, without regard to conflict of law principles. Any disputes will be resolved exclusively in the state or federal courts of Broward County, Florida.
14. Force Majeure
We are not responsible for delays or failures caused by events beyond our control, including acts of nature, network outages, pandemics, government restrictions, or supply chain interruptions.
15. SMS / Text Messaging Policy
15.1 Program Overview
Revyval Health uses SMS messaging to communicate appointment confirmations, reminders, lab updates, treatment recommendations, and support communications.
15.2 Opting Out
Text STOP to unsubscribe. You may rejoin at any time by texting START.
15.3 Data Protection
Your SMS opt-in data will never be shared with third parties except for necessary messaging providers.
15.4 Message Frequency & Rates
Message frequency varies by engagement. Standard message and data rates may apply.
15.5 Compliance
We comply with the Telephone Consumer Protection Act (TCPA) and 10DLC messaging regulations. Our SMS content will never include SHAFT-related (sex, hate, alcohol, firearms, tobacco) or illegal material.
16. Medical Emergencies
Revyval Health does not provide emergency services. If you experience severe symptoms, chest pain, suicidal thoughts, or other urgent conditions, call 911 or go to the nearest emergency room.
